Caring for an ageing population
One of Caring Together’s priorities is to provide joined-up services that help people with long-term conditions to live independently at home when it is safe to do so. Thus includes caring for the needs of an ageing population.
Health and social care partners are invited to an event being held by Cheshire End of Life Partnership on 4 April 2017 from 1.30pm to 3.30pm to advise on responding to the needs of an ageing population.
The session in Worleston Village Hall, Main Road, Nantwich will focus on:
- supporting people with advanced dementia and those important to them
- asset-based community development (i.e. making the best of people, buildings, public and voluntary services to help vulnerable, frail or older people to live well at home).
