Caring Together – A quick reminder

With another New Year upon us, we thought it would be useful to remind readers what Caring Together is all about.

The programme’s vision is: “Joining up local care for all our wellbeing.”

The aim is to encourage self-care by living the values of empowerment (giving people the support they need to look after themselves), collaboration (health and social care partners working together) and innovation (coming up with good ideas to improve the way things are done).

The eight Caring Together ambitions are:

  • People are empowered to take responsibility for their health and wellbeing
  • Access that is designed to deliver high quality, responsive services
  • Appropriate time in hospital, with prompt and planned discharge into well organised community care
  • A prompt response to urgent needs so that fewer people need to access urgent and emergency care
  • The highest quality care delivered by the right person regardless of the time of day or day of the week
  • Carers are valued and supported
  • Simplified, planned care on pathways delivered as locally as possible
  • Staff working together with the person at the centre to proactively manage long-term physical and mental health conditions.
